The one sign of kidney disease that hides in plain sight.

Chester was seven. Golden retriever, gentle to a fault, the kind of dog who followed me from room to room. He'd been standing at his water bowl a little longer than usual. I thought he was being fussy about the temperature — it had been a warm week — and I refilled it. Twice a day. Then three times.

The vet caught it at his annual. Blood panel, then a urinalysis, then a phone call two days later. Chronic kidney disease. Early stage, but present. She said the words gently, the way vets do when they know the owner is about to google everything they shouldn't.

I sat in the car in the parking lot and realized something that has kept me up at night ever since: the sign had been there for months. I had watched it happen. I had refilled the bowl. I had told myself he was thirsty because it was warm. And that quiet increase in his drinking — the thing I never mentioned to the vet because it did not seem worth mentioning — was the earliest signal his body could send me.

What the veterinary literature actually says

Chronic kidney disease in dogs is not rare. It is one of the most common progressive diseases in aging dogs, and by the time most owners bring their dog in for a symptom that finally worries them, the disease has already been running quietly for a long time.

O'Neill et al., 2013. A landmark epidemiological study across 89 UK veterinary practices covering 107,214 dogs found an apparent prevalence of chronic kidney disease of 0.21%, with an adjusted true prevalence of 0.37%. Median survival post-diagnosis: 226 days.

That survival number is the one I could not stop thinking about. About seven months, from the day the vet tells you, to the day you say goodbye. And that is only after diagnosis — which usually happens well after the disease has begun.

Frontiers in Veterinary Science, 2025 (US real-world evidence). More recent US data suggests the number is likely higher than historical UK figures indicated: up to 3% of the general dog population and up to 10% of hospitalized dogs carry chronic kidney disease. Mean age at treatment was 11.4 years — but the underlying disease began years earlier.
Frontiers, 2025 (owner-reported study). When researchers asked owners of dogs later diagnosed with CKD what the first change they noticed had been, the #1 answer was polydipsia — increased thirst. It was named by 14.9% of owners as the earliest observed clinical sign.
Papadakis, 2025 (IRIS-staged cohort). In a study grouping dogs by kidney-disease stage, polyuria (excessive urination) was present in 51.2% of dogs and urine specific gravity declined significantly across stages (F = 12.51, p < 0.001) — a measurable, progressive loss of the kidney's ability to concentrate urine.

Read those numbers together and one story emerges: the disease begins with the kidneys losing their ability to concentrate urine, the dog compensates by drinking more, and the owner — because the sign looks like health — refills the bowl.

Why "drinking more" hides the disease

Here is the paradox that broke my heart when I understood it.

A thirsty dog looks healthy. A dog that trots to the bowl and drinks eagerly is doing something a good dog is supposed to do. When Chester started drinking more, I felt reassured — the way you feel reassured watching a child finish their vegetables. Hydration is health. Everyone knows that.

What actually happens: as the kidneys lose their concentrating ability, more water passes through the system unretained. The body compensates by triggering thirst. The dog drinks more to make up for water it can no longer hold. From the outside, this looks like the same behavior as a dog that is hot, active, or simply thirsty. From the inside, the kidney has already been losing function — often for months, sometimes longer.

By the time the other signs arrive — a slower morning, a little less appetite, the odd bad breath, a small weight change you almost blame on the season — the disease has usually reached stage 3. That is when most dogs are diagnosed. That is where the seven-month median survival number comes from. Not because the disease is fast, but because it is finally visible.

The window of intervention is largest when the dog looks healthiest.

What makes hydration the modifiable factor

You cannot rewrite your dog's genetics. You cannot slow the calendar. There is a lot about how his kidneys age that is outside your hands.

The bowl is not.

The bowl is the single variable in his daily life that you completely control — and it turns out to be a variable that quietly does more than owners realize. Dogs are surprisingly discerning about water. If it tastes off, smells off, or has warmed and sat on the counter all afternoon, many dogs will simply drink less. Not zero — just less. Just enough less that you would not notice from across the room.
